Referrals

To receive an assessment at Sparrow, you need to be referred by one of your health care providers. This allows you to be certain we can provide you with the best evaluation for your circumstances.

Referrals are usually made by clinicians such as:

  • Neurologists. For example, you may have reported concerns about your memory or paying attention after a head injury. They may refer you to determine if this can be seen on testing.

  • General Practitioner. You may have discussed concerns with worsening memory with your family doctor. They may refer you to identify the possible cause.

  • Psychologist or psychiatrist. You may have raised concerns about your thinking with your therapist, who suggests you have your thinking formally evaluated as a result.

Common referral questions include:

  • Memory concerns or suspected dementia

  • Cognitive impacts of neurological conditions (e.g., stroke, MS, brain injury)

  • Clarification of diagnosis when cognitive symptoms are present

  • Baseline cognitive evaluations

  • Return-to-work or school readiness following brain-related conditions.

Please note we do not typically see patients whose primary concern is epilepsy. Such patients should be referred to an epilepsy program, such as the one at Vancouver General Hospital.

  1. Fax it to us at (236) 259 5318.

  2. Send via E2E encrypted email to referrals@sparrow.clinic from a free protonmail.com account.

  3. Call us at (236) 501 5099.

Detail on what to Include

The referral form contains the following.

  • The reason for referral and the specific question(s) you’d like addressed.

  • Relevant medical history and neurological imaging reports (if available).

  • Patient contact information (phone and email).

We will contact the patient directly to schedule an appointment.

Wait Times & Urgency

Most assessments are scheduled within 1-3 months, depending on urgency and complexity. If your referral is time-sensitive (e.g., related to return to work or upcoming medical decisions), please note this in your referral.
